Medical Records

Klamath-Lake CARES requires the following items to release a copy of our evaluation report:

  • Release of Information signed by a legal guardian
  • Completed and initialed mental health section on the Release of Information

The following professionals may request a report:

  • Oregon Department of Human Services (DHS)
  • Law Enforcement
  • Primary Care Doctors
  • Mental Health Therapists
  • Attorneys
  • Judges of the assigned court

If you are a legal guardian of a child that was seen at Klamath-Lake CARES, you should have received a after visit summary after your appointment. If you need another copy of the after visit summary, please contact us at 541.274.7234.

If you are an attorney and would like copies of all medical records, we require a Subpoena Duces Tecum (Records in Lieu of Appearance) and a signed protective order. We can accept subpoenas and signed protective orders via fax or email for your convenience; originals via mail or personal service is not needed. A minimum notice of 5 business days is required.

If you require one of our providers to testify in court, you must serve the provider in-person. We recommend you schedule a specific time to serve the provider, due to varying schedules around patient care. To schedule a time, please call the main line at 541.274.6289.
